Workers’ Compensation at Indiana University

IU provides benefits to employees who sustain injuries by accident arising out of and during the course of their employment.

Injuries must be reported on the day of occurrence or in a timely manner.

Employees should check with their supervisor before seeking medical care except in emergency cases.

Actions that must be taken if you are injured on the job

Identify an authorized medical provider

If you are injured on the job, you must visit an authorized, campus-specific treatment location (this list includes Emergency and/or non-regular business hour options).

Your supervisor must complete the Employer Notification for Treatment form

This form authorizes the initial treatment at the medical provider and needs to be completed before or at the time of treatment.

Your supervisor must complete the Occupational Injury/Illness Report form

Upon receipt of information that an employee has incurred an on-the-job injury, the supervisor must report it and submit this form to the Workers’ Compensation office within 24 hours regardless of whether professional medical attention or lost time is indicated.

NOTE: IU Health Bloomington Occupational Services has moved to 482 S. Liberty Drive, next to the west-side Kroger. It is sharing the location with IU Health Urgent Care.
